Photography and Video Consent Form for bp pulse Events *

I, the undersigned, hereby grant permission to bp pulse and its affiliated program, Music Workshop, to take and use photographs and/or videos of me (or my child, if applicable) during bp pulse events and activities. I understand that these images or videos may be used for the following purposes: • Promotional materials related to bp pulse and Music Workshop • Social media platforms • Websites and online content • Printed marketing materials • Event documentation and future promotions I agree to the following: 1. I will not receive compensation for the use of these images or videos. 2. bp pulse and its affiliates may use the images or videos across various media platforms indefinitely, for promotional, marketing, and archival purposes. 3. bp pulse will have the full rights to edit, publish, or distribute the images or videos as necessary for its communications. 4. I retain the right to request the removal of any specific image or video from future use, by submitting a written request, and bp pulse will make reasonable efforts to accommodate this request. By ticking the box below, I acknowledge that I am over eighteen years of age (or the legal guardian of the minor named below) and have the full authority to grant this consent.
